	IMPORTANT NOTICE: As of 11/01/05, to all who have received funding from the Wisconsin Agricultural Growth Foundation, please be advised that an evaluation of your project or program is required to be presented to the Foundation within 30 days of its completion. If you still have not completed your program or if your project is not complete, we are requesting that you submit an evaluation up to the point of where you are in its completion. The evaluation form can be downloaded from our website, and it is very important that receipts for your spending should be attached to the evaluation. Unfortunately it has come to the attention of the Foundation that some of the organizations and/or individuals who received funding have not used the proceeds of the grant in a manner consistent with their application for a grant.To this end, the Board will not consider any new funding request from anyone who has not submitted an evaluation.
